At Easterseals PORT Health (ESPH), we’re seeking compassionate and dedicated Intensive In Home Therapy Services-Qualified Professional to join our team in Raleigh, NC. What You’ll Do
The In-Home Therapy Services (IHTS) Qualified Professional provides case management and care coordination to support individuals in various environments, including homes, schools, shelters, detention centers, and other community settings. Working alongside the IHTS Therapist, the QP reinforces therapeutic interventions, helps manage psychiatric and addiction symptoms, and delivers crisis intervention to reduce future risks. Responsibilities include linking clients to vital community resources, providing life skills training, supporting caregivers in implementing home-based strategies, and coordinating rehabilitative services to prevent out-of-home placements. Additionally, the QP may assist in crisis response, peer reviews, and new employee training while ensuring compliance with agency and service model requirements.

Hours will range from Monday-Friday, must be available for a flexible schedule that may require afternoon and evening sessions, some possible weekend sessions. 24/7/365 on call rotation every 3 weeks.

What We’re Looking For:
To join our team, you must have passion for working with youth and families, ability to enjoy a flexible schedule and do what it takes to meet families where they are at, enjoy driving, creative and innovative in able to meet children and families needs, work independently and be a team player.
We also require the following:
- Bachelor’s degree in human service field with two years post degree experience with population served (at risk youth); or
- Master’s degree in human service field with one year post degree experience with population served (at risk youth). Must have documented experience with the population served
- Must be able to operate a motor vehicle and have a valid insurance and an unrestricted driver’s license
