About Us: Eastside Mental Health is a mental health center serving Jefferson, Blount, and St. Clair Counties. With locations in Birmingham, Pell City, and Oneonta, we provide services to over 2,000 clients who are managing a mental health condition. Our professional staff includes therapists, nurse practitioners, and medical doctors who are supported by our administrative team.
We are seeking a compassionate and experienced Case Manager to join our team. The ideal candidate will provide direct support to individuals and families, empowering them to achieve their goals and improve their overall well-being. This role requires a strong understanding of behavioral health and social work principles, as well as the ability to navigate complex situations with sensitivity and discretion. The Case Manager will work collaboratively with clients, healthcare providers, and community resources to deliver comprehensive services that address a wide range of needs.
**Responsibilities**
- Provide case management services to individuals and families, including assessing needs, assist with developing individualized treatment plans, and monitoring progress.
- Encourage clients to take an active role in their healthcare and personal growth.
- Collaborate with clients to identify barriers to care and develop strategies to overcome them.
- Utilize crisis management and crisis intervention techniques to address immediate needs and ensure the safety and well-being of clients.
- Maintain accurate and timely records of client interactions, assessments, and treatment plans that comply with agency and Department of Mental Health standards
-Transport clients to services at various locations in the community
-Provide clinical services in the community, client's home and/or office including: crisis intervention, basic living skills, and family support/education
- Participate in regular team meetings to share best practices, discuss client progress, and address any challenges or concerns.
-Adhere to policies regarding client protection, rights, abuse, and incident reporting
**Minimum Qualifications**
- Bachelor's degree in social work, psychology, or a related field.
- Two years experience (paid or volunteer) in case management, preferably with adults with a mental illness
- Strong understanding of mental illness and community mental health practice
- Excellent communication and interpersonal skills.
- Ability to work in a fast-paced environment and adapt to changing circumstances.
- Strong analytical and problem-solving skills.
- Ability to maintain confidentiality and adhere to HIPAA regulations.
-Willingness to work in a non-traditional and in-home settings
-Posses an unrestricted Driver's License and personal vehicle for the purposes of transporting clients. Mileage is reimbursed by the agency.
Job Type: Full-time
Pay: $33,000.00 - $43,000.00 per year
