Job Summary
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN/RN) to join our healthcare team. The LPN/RN will provide essential nursing care to patients in various units in the home. This role requires a strong understanding of patient needs, effective communication skills, and the ability to work collaboratively with healthcare professionals to ensure high-quality care.
Responsibilities
- Administer medications and treatments as prescribed by physicians.
- Monitor patients' vital signs and report any changes to the healthcare team.
- Assist in the development and implementation of patient care plans.
- Document patient progress and maintain accurate medical records in compliance with standards.
- Collaborate with other team members and other healthcare professionals to optimize patient outcomes.
- Participate in behavioral health assessments and interventions as needed.
Qualifications
- Valid LPN/RN license in the state of practice.
- Knowledge of anatomy and medical-surgical nursing principles.
- Experience working with geriatric patients is preferred.
- Strong understanding of aseptic techniques and infection control standards.
- Excellent interpersonal skills for effective communication with patients, families, and staff.
- Ability to work well under pressure in a fast-paced environment while maintaining attention to detail.
- Previous experience in post-acute healthcare settings is advantageous but not required.
