Key Responsibilities:
- Resident Assessments: Conduct thorough evaluations of residents' physical, cognitive, and psychosocial status using the MDS 3.0 assessment tool.
- Care Plan Development: Collaborate with interdisciplinary teams—including nursing, physical therapy, nutrition, and social services—to create individualized care plans based on assessment findings.
- Regulatory Compliance: Ensure all assessments and care plans comply with state and federal regulations, including the Resident Assessment Instrument (RAI) process.
- Quality Monitoring: Participate in quality assurance and performance improvement initiatives to enhance resident care and facility operations.
- Staff Education: Provide training and guidance to staff on MDS assessments, care planning, and documentation standards.
- Family and Resident Communication: Engage with residents and their families to discuss care plans, address concerns, and provide updates on resident status.
Qualifications:
- Education: Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.
- Experience: Prior experience in long-term care settings, with a strong understanding of the MDS 3.0 process and RAI guidelines.
- Certifications: Certification as a Resident Assessment Coordinator (RAC-CT) is preferred.
- Skills:
- Strong analytical and critical thinking abilities.
- Excellent communication and interpersonal skills.
- Proficiency in electronic health records (EHR) and MDS software.
- Detail-oriented with a commitment to accuracy.
