We are seeking a skilled Rehabilitation Services Coordinator to join our team of dedicated healthcare professionals.

Key responsibilities include reviewing patient applications, maintaining accurate records, and communicating effectively with hospital discharge planners.
A basic understanding of insurance, electronic medical records, and medical terminology is preferred.
This role will also involve learning the basics of Minimum Data Set (MDS) and assisting the Care Coordination Group with other duties to complete team goals.
Responsibilities:
- Review patient applications and maintain accurate records
- Communicate effectively with hospital discharge planners
- Ensure timely completion of referral information and data prior to an admission arriving
- Enter patients in the Electronic Medical Record (EMR)
- Follow up daily on pending referrals
- Bridge communication with the team regarding new admissions
- Basic understanding of insurance, electronic medical records, and medical terminology preferred
- General use of Microsoft Office Tools
