Selects, adapts and administers appropriate evaluation procedures to assess patients level of performance, including abilities and areas of impairment, administers tests according to standardized techniques.

Develops short and long term goals based on physician referral, evaluation and patient goals; reevaluates patients at appropriate intervals changing treatment goals as indicated.

Participates in and directs the provision of services for patients according to age specific needs, utilizes appropriate modalities to meet established goals, selects treatment environments and activities according to individual patient needs and monitors progress and treatment to modify therapeutic goals.

Completes patient charges accurately and in a timely manner.

Ensures appropriate and legible documentation of assessments, recommendations, progress notes and discharge plans are made on the medical record according to hospital and departmental policy; contacts physician with referral or treatment questions or concerns.

Provides patient/family/caregiver education and verifies their understanding of instruction.

Ensures all services delivered are consistent with professional standards recommended by American Speech & Hearing Association; maintains current knowledge of Speech & Language Pathology and keeps management informed of new developments.

Works closely with other therapists and disciplines to coordinate care of the patient and ensure smooth transition of care between staff members.
