Case Manager Job Description
We are seeking a skilled Casual Case Manager to join our team at Eaton Rapids Medical Center. This is an exciting opportunity for a dedicated professional to make a difference in the lives of our patients.
Responsibilities
- Conducts admission and continued stay reviews for medical necessity, severity of illness, and intensity of service.
- Coordinates proper admission status with physicians and obtains authorization for reimbursement in a timely manner.
- Develops and maintains a collaborative team to evaluate and create processes for denied authorizations.
- Provides quality psychosocial needs to patients and their families by formulating diagnostic impressions.
- Oversight and coordination of all patients admitted to swing bed programs.
- Updates policies on case management, utilization review, and swing bed programs.
Requirements
To be successful in this role, you will need:
- BLS certification upon hire and maintained throughout employment.
- A Master's degree in Social Work or a graduate of an accredited school of nursing and current licensure to practice in the State of Michigan.
- 2-3 years of experience in case management and/or utilization review.
