Job summary
Cooperative Health is seeking an enthusiastic, mission driven individual to fill the position of Family Medicine Physician. Under general supervision of the CMO, the qualified candidate will serve as a key member of the Eau Claire Cooperative Health Center's multi-disciplinary health care team. Primary, preventive, and episodic care is compassionately delivered to assess each patients unique health status and promote health and wellness. The clinical program is divided into units that foster a team approach to care. The Physician will assume an active role at the site becoming involved with quality improvement activities, meetings, and other team building efforts.
Essential job functions
Administrative
- Completes medical records in a timely manner as specified by the medical record completion policy.
- Responds to messages from support staff and other members of the office team.
- Attends Cooperative meetings as scheduled (75% of all required staff meetings)
- Be knowledgeable of and work with care team to coordinate plans to improve population health (quality measures)
- Achieve expected targets with quality measures (UDS, Hedis, etc).
Clinical
- Obtain a through medical history, conduct a physical exam (as appropriate), and complete an assessment and plan.
- Make appropriate and timely medical diagnosis based upon history and physical exam.
- Orders appropriate test to confirm or evaluate the medical situation.
- Prescribes medications and give detailed instructions for administration.
- Provides encounter information necessary for billing purposes (i.e. indicating appropriate CPT and ICD-10 codes for services rendered).
- Interprets and makes sound clinical decisions in a timely manner, monitoring the effectiveness of care plans
- Directs coordination of patient care with patient and other members of the clinical team.
- Successfully engage, educate, and empower patients to take control of their personal health.
- Provides medical supervision to support staff in implementing the plan of care.
- Serves as a preceptor for professional students during clinical rotations at Cooperative Health.
- Establishes an effective working relationship with public and private clinicians and other clinical care referral sources.
- Work the Saturday Walk-In office as arranged by the Division Director and / or CMO.
- Performs other duties as assigned by the CMO and CEO.
Qualifications
- Must have successfully completed medical training at a (4) year, accredited medical school and received a M.D. degree
- Board certification preferred
- Must be a licensed physician in good standing
- Must be Board eligible
- Must maintain required amount of CMEs to meet licensure and specialty board requirements
- Must maintain DEA and controlled substance registration in good standing
- Current BLS certification, if ACLS, PALS, or NALS certified for BCLS will be waved
