Position Summary: Cooperative Health is seeking an innovative, enthusiastic individual with strong customer service skills, case management, communication, and Microsoft application skills (e.g. Excel spreadsheet) will be suitable for this role. The DPSS position will involve understanding of the following including systematic, data-guided initiatives or processes designed to improve clinical care, patient safety, health care operations, services, and programs or form developing new programs or services (e.g. teaching evaluations, patient /employee service surveys, researching clinical trials for program use, etc.
Principles Responsibilities:
- Responsible for continuous quality improvement projects to ensure the development and implementation of activities to make changes to the program in response to the performance data results.
- Ensures documentation of core and support services to mental health services, substance abuse outpatient care, health education/risk reduction and other area are entered into CAREWare.
- Coordinates and manage the development, implementation and monitoring of clinical trials per ongoing reporting requirements.
- Conducts routine assessments using standardized quality management assessment tools including patient surveys.
- Documents all medical transportation services provided in Athena and CAREWare.
- Compiles and maintains summaries of minutes for departmental meetings.
- Orders departmental office supplies, as needed.
- Performs other duties as assigned
Education & Experience:
- High school diploma or equivalent and four years of work experience in a medical care setting is acceptable.
- Master’s degree preferred from an accredited college or university in Social Work or Counseling or Public Health or Health & Human Services Field. Bachelor’s degree accepted
- At least two years working in a clinical/community setting and/or successful community outreach/recruitment and volunteer coordination, or any equivalent combination of training and experience.
- Spanish speaking is a plus.
- Must be able and willing to drive to all four counties (Fairfield, Lexington, Newberry, Richland) covered by Cooperative Health.
- Must have reliable transportation.
- Must have the ability to understand and respond effectively and with sensitivity.
- Experience working with diverse communities to address healthcare needs.
- Federally Qualified Health Center (FQHC) experience preferred demonstrated proficiency and comfort with technology; able to use MS Office (e.g., Word, Excel, Outlook, and PowerPoint), database management, webinar systems, and HTML email systems effectively and efficiently.
