Full time position (30-40 hours/week)
Primary Job Purpose: To address the social, psychosocial, and mental health needs of the
neighbors and to enhance their quality of life in Assisted Living.
Essential Job Functions:
- Practices the Values, the Mission, and Philosophy of Eben Ezer Lutheran Care Center.
- Shall provide orientation to new neighbors and their families (including explanation of residents’ rights, advanced directives, house rules, etc.)
- Responsible for planning activities, coordination, ordering supplies and ensuring activities are scheduled and meeting the needs of neighbors.
- Will assist with activities as needed.
- Responsible for training and mentoring Assisted Living aides.
- Responsible for assessing and monitoring neighbors’ psychosocial, financial, and mental health needs, reporting, and completing required paperwork to proper agencies and individuals as necessary.
- Will serve and assist neighbors and families with mood and psychosocial needs, specifically in the areas of family and/or resident resolution of issues.
- Will assist with admission screening, admission processes, and discharge planning.
- Will assist neighbors and families with other issues, as needed.
- Will provide accurate and timely documentation and update care plans. Will monitor daily progress notes to stay current on the needs of neighbors.
- Responsible for attending psychotropic drug review meetings and presenting necessary information on neighbors.
- Assist with incident reporting and abuse/neglect investigations.
- Communication of neighbors’ condition & significant issues with physicians, families, staff and Administrator as necessary.
- Will assist the department in survey preparations and regulatory compliance.
- Will assist with personal care, medication administration, bathing, and housekeeping as needed.
- Coordinate transfers (room changes) within the community and assist neighbors in adjusting to the transfer.
- Assist neighbors with accessing outside resources and financial management, such as applying for Medicaid, when the family is not available.
- Obtain necessary knowledge of State and Federal regulations.
- Assist Assisted Living Administrator with Quality Management initiatives.
- Assist with neighborhood meetings, other activities, and other large community-wide events, as needed.
- Must maintain confidentiality of all work-related information according to HIPAA regulations. Assure neighbors’ rights, privacy, and HIPAA regulations are followed.
- In addition to the responsibilities described above, the role may include other responsibilities and duties as assigned on occasion, based upon Eben Ezer's needs or requirements
Qualifications
Qualifications: To perform this job successfully, an individual must be able to perform each
essential duty satisfactorily. The requirements listed below are representative of the
knowledge, skill, and/or ability required.
Qualifications, Education, and Experience: Bachelors Degree in Human Services or related field
or equivalent experience is desired. Healthcare experience in a geriatric setting preferred.
Must possess a valid Colorado driver’s license. Must obtain QMAP certification within 6 months
of acceptance of position.
Non-Essential Job Functions: May serve on Eben Ezer committees. May need to assist with
neighbors’ transportation to appointments. Will attend meetings and department meetings as
instructed.

- Knowledge of medical terminology.
- Experience with Medicare and Medicaid regulations.
- Experience with long-term care, working with the elderly, knowledge pertaining to dementia and related conditions.
- Ability to work independently in completing work assignments and manage time efficiently. Ability to multitask and work through stressful situations.
Material and Equipment used: General office equipment, such as phone, fax, copier, adding
machine/calculator, scanner, shredder and computer.
Working Environment/Physical Activities: Must be able to sit, stand, and walk 8 hours a day.
Must be able to lift and carry 0-50 pounds regularly and 50 pounds occasionally with
assistance. Must be able to push/pull up to 250 pounds regularly using a wheelchair. Sits,
stands, bends, walks, reaches, lifts, keyboarding, pushing wheelchairs, and moves
intermittently during working hours. Employee is subject to physically ill neighbors as well as
hostile and emotionally upset neighbors and/or families.
Working Hours: General office hours Monday - Friday, some evenings and weekends.
Occasional help covering as a QMAP or aide on the neighborhoods may be required. Schedule
may vary, based on needs and supervisor discretion.
Working Location: This is not a remote position and work must be performed in-person.
