Ebenezer Medical Outreach, Inc. is seeking a Community Health Worker to help change lives in our community. Working under the direction of our Nurse Clinic Coordinator, you will become an important member of our interdisciplinary team, connecting our patients to resources that will empower them to transform their health and enrich their lives. You will meet patients where they are – often literally in their homes – to build life-changing relationships through your creativity, dependability, and compassion.
The primary functions of the CHW are:
- Provide social interaction, emotional support, and help applying disease self-management plans to high-risk patients in the context of their home and community
- Link individuals with clinical, community and other resources and serve as a liaison between patients and the clinical care coordination team
- Provide ongoing, flexible, accessible support for patients as their needs change
- Coordinate community outreach events and educational programs
- Complete necessary communication, documentation, reporting, and billing activities in an accurate and timely manner
- Safely and legally operate a personal motor vehicle to meet patients in their homes and participate in offsite activities.
Scope of Practice
- Works under the direction of a medical provider.
- Assists patients in self-management of chronic illnesses and medication adherence as directed by the medical provider.
- Acts as a bridge between individuals, families, communities and health and human services.
- Provides culturally appropriate health education, information and outreach in community-based settings, usually in the patient’s home.
- Provide services, such as advising on access to health and human services, social support, care coordination and health screenings.
Expectations
- Empower and transform our patients with chronic health conditions through gaining access to hard-to-reach individuals and patiently coaching and supporting them toward achieving their health goals
- Provide referrals and services under the supervision of a licensed provider or Registered Nurse (CHWs do not provide patient care).
- Identify and address social determinants of health that are impacting community outcomes
- Help to decrease costs, inappropriate Emergency Department visits, and health disparities
- Fulfill other duties as assigned for program and patient success.

Minimum Education / Experience
High School diploma or GED required. Two years of health/clinical experience or equivalent health-related education required. CPR certification required within first 30 days of employment. Knowledge of community resources and experience working with a variety of community-based organizations preferred. Demonstrated ability to use Microsoft Office, electronic medical records (EMR), and various other software programs is required. Current driver's license, motor vehicle safety inspection certification, and proof of insurance are required.
Working Environment
This position generally functions within a health care office setting with frequent time spent on the computer, or interacting with clients, patients, families, and other health care team members via telephone, electronically, or face-to-face. This position also involves working in the community, including frequent travel within the immediate counties and cities served by Ebenezer Medical Outreach.
