POSITION SUMMARY :

In cooperation with Nursing Administration, the charge nurse position ensures that a consistently high-quality level of care is delivered throughout the nursing facility. The position requires advanced training and broad nursing experience including supervisory duties. The work requires administrative ability, knowledge of training techniques, ability to adapt techniques and procedures in accordance with unusual situations and the exercise of initiative and good judgment. Wound Charge Nurse also provides direct care, administer treatments and medications, responsible for wound rounds and documentation, tracking and update each resident's comprehensive resident assessment and plan of care.

RESPONSIBILITIES / ACCOUNTABILITIES :

Knowledge of geriatric and general chronic care nursing principles and professional standards of nursing practice and ability to apply to resident specific circumstances. Able to identify, implement and evaluate appropriate objectives and interventions for residents.

Performs skin checks on all residents daily.

Accompanies NP on wound rounds weekly and updates notes in PCC.

Tracks wounds in facility and reports to DON / ADON

Carry out wound orders.

Review new / re-admit charts, care plans, orders, labs and documentation.

Inservice staff on treatment pass

Assist with treatments.

Audit treatment and medication carts and ensure they are kept up weekly.

QAPI for wounds

Collaborates with Nursing Supervisor daily, to exchange information regarding resident care, resident condition and all matters affecting resident well-being (i.e., admission, discharges, transfers, out of or within the building), and makes recommendations regarding nursing process.

Verifies unit staffing as necessary.

Makes rounds, morning and night, as time allows, to assess residents condition, giving special attention to critically ill residents.

Ensures that nursing staff are making the required rounds in visiting each resident.

Ensures that all residents’ call lights are answered as promptly as possible by nursing personnel.

Ensures that the individual nursing care plan is followed.

Reinforces the training and orientation of nursing personnel and provides positive reinforcement.

Admits, transfers, and discharges all residents according to policy.

Contacts the attending physician for initial orders on each admission unless these orders accompany the resident in writing and are currently dated and signed by the physician.

Oversees residents’ medications, which includes seeing that refills are ordered as necessary and that all medications are handled in accordance with the written policy on medications.

Performs treatments and dispenses medications, as ordered and in accordance with Windsor Gardens Care Center policies and procedures.

Ensures that the residents are receiving specially ordered diets, where applicable.

Reports all problems and incidents as soon as possible to the Director of Nursing Services (D.O.N.). In the absence of the D.O.N., these are reported to the Asst. D.O.N., or Administrator.

Arranges for resident’s hospitalization when so ordered by the physician. Also notifies the resident’s relatives of such need for hospitalization.

Assists with implementation of facility Quality Improvement program.

Oversees charting process for each resident.

Assumes responsibility for the identification of potential safety hazards and for the identification of noncompliance with facility guidelines as set forth in the Policy and Procedure Manual.

Ensures that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals’ needs and rights.

Performs other duties as requested.

SPECIFIC EDUCATIONAL / VOCATIONAL REQUIREMENTS :

The Charge Nurse must be a graduate of a state-approved school of nursing and currently registered as a registered nurse in the state, or a licensed practical nurse that is a graduate from a state-approved school of practical nursing and currently registered in N.J.

Up-to-date physical assessment skills and comprehensive knowledge of nursing principles are also required.

Recent experience in geriatric or rehabilitative nursing and experience in supervisory capacity are preferred.

Prior wound care experience

Must have thorough knowledge of nursing theory, technique, and practices.

Must have thorough knowledge of the specialized procedures applied and equipment used in the area to which assigned.

Must have knowledge of dietetic, sanitation and personal hygiene.

Must have knowledge of medications, their proper dosage and expected results.

Must have skill in handling and manipulating instruments and equipment.

Must be able to coordinate and supervise the nursing functions in an effective manner.

Must be able to maintain an empathetic attitude toward residents and to gain their confidence to work with them effectively.

Must be able to understand and follow oral and written instructions of a technical nature.

Must be able to train subordinates in work procedures and to organize and direct training carried out by members of the staff.

Must possess the ability to positively interact with personnel, residents, family members, visitors, government agencies / personnel and the public.

Must provide annual verification of a negative TB skin test.

PERFORMS RELATED DUTIES :

Interacts with residents, families, visitors, and employees.

He / she carries out other tasks as requested in situations where hands-on intervention / participation may be required.
