Job Summary: We are seeking an Admissions Nurse to provide high-quality nursing care to patients in their homes. This role requires effective communication, teamwork, and a commitment to maintaining high standards in patient care.
Key Responsibilities:
- Patient Care: Admit/readmit patients, assess physical and psychosocial needs, develop care plans, ensure safety, and evaluate patient progress, including discharge planning.
- Documentation and Compliance: Maintain accurate documentation, ensure care plans align with industry standards, and comply with regulations.
- Case Management: Manage work assignments effectively, visit new patients within 24 hours, report at-risk situations, and delegate tasks to home care nurses.
- Home Health Aide Supervision: Develop and evaluate care plans for aides, supervise them regularly, and ensure compliance with regulations.
- Professional Development: Stay current with professional skills, attend required training, and adhere to agency policies like HIPAA.
- Team Collaboration: Work effectively with other healthcare providers, offer guidance to new nurses, and provide support as needed.
Requirements:
- Graduate of an NLN-approved school of nursing, associate or baccalaureate degree program.
- Possession of a current license to practice as a Registered Nurse in the State of Connecticut.
- One (1) year of experience in community health nursing home care or hospice is required.
Skills and Abilities:
- Knowledge of professional nursing theory and practice.
- Ability to apply and modify the principles, methods, and techniques of professional nursing to provide ongoing patient care.
