Admissions Nurse Job Summary:
The Admissions Nurse role at ECHN involves providing intermittent nursing services to patients in their homes, developing care plans in collaboration with physicians while adhering to agency policies.

Responsibilities:
- Provides intermittent nursing service to patients in their homes under a plan of care established in collaboration with the patient#s physician and according to agency policies and procedures.
- Admits clients to service, establishes a plan of care, and refers to other disciplines and agencies as necessary.
- Develops and evaluates care plans for home health aides, supervises them regularly, and ensures compliance with regulations.
- Maintains accurate OASIS documentation, ensures care plans align with industry standards, and complies with regulations.
- Manages work assignments effectively, visits new patients within 24 hours, reports at-risk situations, and delegates tasks to home care nurses.
- Works effectively with other healthcare providers, offers guidance to new nurses, and provides support as needed.
Requirements:
- Graduate of an NLN-approved school of nursing, associate or baccalaureate degree program.
- Possession of a current license to practice as a Registered Nurse in the State of Connecticut.
- One (1) year of experience in community health nursing home care or hospice required.
