As a trusted behavioral health organization committed to providing care in the community since 1955, we offer an array of comprehensive services for youth, adolescents, and adults. Our Community Support Team program provides intensive crisis case management services and crisis support for adults in the community.
Primary Responsibilities
- Assist adults with persistent or recurrent mental illness who live independently in the community via locating housing, food & transportation resources, applying for entitlements, providing counseling and life skill training, facilitating the development of natural community supports, and advocating for the client's needs.
- Spend 50% of work hours providing direct client centered recovery-oriented services
- Develop goal oriented IATP’s that are strength-based, and recovery oriented on all assigned clients as required
- Provide accurate, quality documentation within a maximum of 48 hours after service delivery
- Provide medication monitoring and training regularly and in a timely fashion
- Participate in an on-call CST emergency phone rotation
Performance Requirements
- Demonstrate knowledge of documentation standards and capacity to meet documentation expectations.
- Ability to get clients to participate in individual sessions.
- Ability to assess, diagnose, interpret and integrate both verbal and non-verbal client input during session.
- Ability to clinically “listen” during session.
- Ability to establish rapport with clients and interact constructively with others in the workplace.
- Ability to focus client on individualized treatment plan and goals.
- Give information in a clear and understandable fashion and respond constructively to appropriate client questions and concerns.
Education and Experience
- Bachelor’s degree in human service field or equivalent experience
- One-year clinical experience
- Must have a valid Illinois driver’s license, auto insurance, automobile, and a willingness to transport clients as necessary
Schedule
- Tuesday – Friday, 10am – 6pm & Saturday, 9am – 2pm (35 hours a week).
