Key Responsibilities:
- Evaluates and treats patients/residents.
- Communicates with patients’ families, physicians, and other health team members.
- Maintains all necessary documentation of services in the patient’s medical records.
- Treatment planning and implementation.
- Discharge planning.
- Testing patients’ physical abilities.
- Analyzing rehabilitation goals.
- Consulting with the rehab team and other health team members to coordinate the most effective treatment program.

- A valid Physical Therapist licence in the State of California.
