Position Summary
- The Care Manager is responsible for coordination of the plan of care through collaboration with the provider, the interdisciplinary team, the patient and family.
- The Care Manager facilitates the plan of care with the underlying objective of enhancing the quality of patient care, optimizing clinical outcomes and patient satisfaction while managing the cost of care. In addition, the Care Manager helps drive change by identifying areas where performance improvement is needed (e.g., day to day workflow, education, process improvements and patient satisfaction).
- The Care Manager is accountable for a designated patient caseload and plans effectively in order to meet patient needs, manage the length of service, promote efficient utilization of resources and ensure a well-organized and safe plan of care is established for every patient.
Minimum Requirements
- Education: Associate Degree in Nursing or higher or Bachelors of Science in Social Work or higher is required.
- If a Registered Nurse, current licensure as a Registered Nurse by the NC Board of Nursing is required.
- Experience: 0-3 years of Medical Case Management

- This full-time position (40hrs/week) is with the 2N Progressive Care unit
- Hours: Monday to Friday - 8:00am - 5:00pm
- No weekends or nights
