Position Summary
The School Health Nurse Care Manager is responsible for assessing, planning, implementing, coordinating and evaluating school health services for their designated student caseload.
The School Nurse Care Manager facilitates a plan of care with the underlying objective of; minimizing or eliminating health related barriers that impact learning, maximizing the amount of time in school by reducing the incidence of health-related absenteeism and avoidable ED visits/hospitalization, and assist students in better understanding and managing their chronic health conditions so they can achieve the highest degree of independent functioning possible.
To accomplish these objectives, the School Health Nurse Care Manager serves in the role of care coordinator, case manager, collaborator, advocate, health educator and counselor for their entire assigned school/schools.
The School Health Nurse Care Manager is expected to function independently in their assigned school sites under state and agency guidelines and policies and without the benefit of onsite assistance from other healthcare professionals.
Minimum Requirements
Bachelor of Science in Nursing (BSN) is required
Active NC nursing license
BLS certification
3 years of nursing experience is required. Previous school health and case management experience is preferred.
National Certification with the National Board for Certification of School Nurses (NBCSN) is required within 3 years of obtaining school health nurse care manager position.
Other Information
This is a full-time, 36 hours per week, position.
Primarily weekdays, M-F, 8a-5p with occasional weekend or evening community events required.
This position is not eligible for overtime.
