Position Summary
The Rehab Discharge Coordinator (RDC) position will serve to support the discharge planning processes by coordinating post discharge services within the Medical Center Inpatient Rehabilitation Service line. The RDC may assist in collection of patient information, discharge care needs, regulatory interventions as well as work to facilitate a transition plan that promotes a safe and effective discharge and utilization of services. The RDC will perform other activities related to the discharge planning, rehab admissions and UR process as requested.
Minimum Requirements
Associate Degree in Human Services related field required
BS preferred
Two Years Of Healthcare Related Experience Is Preferred.
Experience in case management, doctor's office, home health agency, DME agency, or health care field is strongly preferred.
Knowledge of medical terminology and basic computer skills are required. Intermediate MS
Word And Excel Skills Preferred.
