Job Description
The Home Health Aide is responsible for providing personal care and other designated health-related services designed to maintain the client’s physical and emotional well-being according to the physician-ordered plan of care.
Specific responsibilities include:
- Follows the client’s plan of care to help each client reach and maintain his or her highest level of performance and independence in the home environment. These services include: bathing, grooming and assisting with activities of daily living.
- Provides communication with client and/or family while in client’s home as appropriate.
- Follows schedule frequency as ordered by physician.
- Proactively participates in care conferences with other health team members as needed.
- Maintains privacy and confidentiality of client, client's records and client's living environment. Understands and follows HIPAA guidelines.
- Promotes personal safety and a safe environment for clients and co-workers.
- Other duties as assigned.
Qualifications
The following are the required minimum qualifications for the Home Health Aide position:
- High School Diploma or GED.
- One (1) year experience as a Home Health Aide, Medicare home health experience preferred.
- Certified Nursing Assistant (CNA) license.
- Active status in the MN Nursing Assistant Registry.
- Valid driver's license, current auto insurance and reliable vehicle.
- Ability to communicate effectively, including written and verbal skills.
- Ability to work well both individually and as part of a team, through respect, collaboration and dedication; successfully interact with a diverse culture base.
- Ability to prioritize and organize work effectively and efficiently.
- Demonstrated positive attitude, high integrity and ethical awareness.
- Proven to be dependable and responsible.
- Compassionate and caring attitude.
