RN Clinical Care Coordinator

We are seeking a highly motivated Clinical Care Coordinator to join our team. This full-time position offers a $12,000 sign-on bonus and competitive pay ranging from $44.45 to $50.45 per hour, based on experience.

• Responsibilities include assessing residents' physical and mental functions, documenting data accurately, and making appropriate referrals to healthcare professionals.
• Supervise resident care, oversee clinical coordination, and manage the infection prevention program.
• Requirements: Registered Nurse (RN), strong customer service, interpersonal, and time management skills.
