Now Hiring: MDS Coordinator (RN)
Edenbrook of Rochester, a skilled nursing facility in the Eden Senior Care Network, is seeking a dedicated MDS Coordinator (RN) to join our team. This leadership role offers the opportunity to oversee clinical assessments, care planning, and reimbursement processes while working alongside a supportive interdisciplinary team.
About the Role
We are looking for a motivated and knowledgeable Registered Nurse with expertise in MDS, RAI, and Clinical Reimbursement. As a key leader, you will be responsible for guiding the team in the following areas:
Key Responsibilities
- Oversee and manage MDS scheduling, setup, and timely completion.
- Handle new admission authorizations, updates, family/insurance communications, and case manager coordination.
- Perform ICD-10 coding for new admissions and ongoing updates for the LTC population.
- Collaborate with the interdisciplinary team (IDT) on developing, updating, and reviewing resident care plans.
- Communicate with families and residents regarding care plans, insurance needs, and discharge planning; participate in care conferences.
- Educate the IDT on MDS requirements, assessments, and skilled care service standards.
- Lead Utilization Review Meetings, ensuring accurate PDPM scores and rates.
- Provide training and guidance to staff to maintain proper documentation standards.
- Participate in the facility’s Quality Measures and QAPI process for continuous improvement.
- Conduct chart reviews and audits as necessary.
- Assist the IDT during surveys and inspections.
Qualifications
- Active RN license in the state of Minnesota
- MDS experience is preferred
- Strong knowledge of:
MDS process
ICD-10 coding and guidelines
Medicare Chapter 8 Manual and RAI Manual
- Excellent organizational, written, and oral communication skills.
- Proficiency with tools like Excel, Teams, or Zoom is a plus!
- Experience with Point Click Care (PCC) is preferred.
