Position Overview:
The Regional Hospital Liaison will serve as the primary connection between designated hospitals and our centers. This role focuses on fostering relationships, evaluating patient needs, and ensuring smooth transitions into our care facilities.
Key Responsibilities:
- Manage multiple designated facilities.
- Conduct daily hospital rounds to evaluate potential high-acuity admissions.
- Assess patient care needs and facilitate a seamless admission process.
- Build and maintain relationships with hospital staff, including social workers, discharge planners, and physicians.
- Communicate discharge plans and patient needs to interdisciplinary teams.
- Promote Eden's services to meet community healthcare needs.
Qualifications:
- Familiarity with local healthcare organizations and hospitals (training available).
- Proven ability to develop professional relationships with hospital staff.
- Sales and marketing experience related to referral generation.
- Existing relationships with social workers and discharge planners are a plus.
- Strong presentation and relationship-building skills.
