The Community Liaison serves as a key representative of Eden's Care, building relationships with the local community, healthcare providers, referral sources, and other organizations to promote services and increase awareness. This role focuses on outreach, networking, and educating the public about the benefits of our services, ensuring that individuals in need can easily access the care and support they require.
Key Responsibilities:
Community Engagement & Outreach:
Develop and maintain relationships with hospitals, physicians, nursing homes, assisted living communities, and other healthcare providers.
Represent the organization at health fairs, networking events, and community meetings to promote awareness and establish partnerships.
Organize and lead educational presentations to inform the public about available services and resources.
Referral Development & Networking:
Identify and cultivate new referral sources while strengthening existing relationships.
Serve as the primary contact for referral partners, ensuring seamless communication and coordination.
Work closely with case managers, discharge planners, and social workers to facilitate referrals and patient transitions.
Marketing & Promotion:
Distribute marketing materials, brochures, and other informational content to increase visibility in the community.
Collaborate with the marketing team to develop outreach campaigns and promotional events.
Utilize social media, networking events, and other platforms to raise awareness about the organization’s services.
Client & Family Support:
Educate individuals and families about eligibility for services and assist in the referral process.
Provide guidance and support to families navigating care options.
Address concerns or questions from prospective clients and ensure they are connected to appropriate resources.
Tracking & Reporting:
Maintain accurate records of outreach efforts, referral sources, and community interactions.
Provide reports on referral trends, community engagement activities, and overall program effectiveness.
Evaluate outreach strategies and make recommendations for improvement.
Qualifications:
Education & Experience
High school diploma required; Bachelor’s degree in Marketing preferred, Communications, Healthcare Administration, or related field preferred.
Previous experience in community outreach, business development, marketing, or healthcare sales preferred.
Experience in senior living, home care, hospice, or healthcare settings is a plus.
Skills & Abilities:
Strong interpersonal and communication skills with the ability to build relationships.
Self-motivated with excellent organizational and time-management abilities.
Comfortable with public speaking and delivering presentations.
Knowledge of community resources and healthcare referral networks.
Proficiency in Microsoft Office and CRM software (preferred).
Other Requirements:
Valid driver’s license and reliable transportation for travel to community events and referral partners.
Ability to work flexible hours, including evenings or weekends as needed for events.
