Payrate: $58/hr

Shift - M-F 8am-5pm with some weekend coverage. Some call may be scheduled, but only on an emergency basis.

Contract:13 Weeks

Guaranteed Hours: 32

Job Description:

EXPERIENCE/EDUCATION/QUALIFICATIONS

-Must have OASIS (Outcome and Assessment Information Set) Experience

-Home Health and Hospice Experience required

-Experience traveling from patient to patient for home visits

-Must be able to manage and adjust to changes in schedule

-Must have drivers license in good standing

-BLS required

-EMR system used is HealthWyse (Careficient)

-Outpatient care
