Duties and Responsibilities
Direct Service
- Serve as case manager for assigned clients which could include activities that focus on the need of the client.
- Provide direct client counseling and support in individual, group setting and involves families whenever possible.
- Functions as contact person with Psychiatrist, Medical Doctor and IPU personnel when patient is referred into or out of units or at other times as necessary.
- Acts as liaison to courts, schools, department of welfare and industries providing reports on treatment of the clients.
- Prepare individual treatment plans at prescribed intervals with the interdisciplinary team.
- Participates in urine surveillance and collects urine upon request.
- Refers, with necessary accompanying information, client for psychological or psychiatric evaluation within the Center or to other treatment agencies.
- Dictates pertinent treatment observations into the patient record at prescribed intervals and completes all necessary administrative forms pertaining to the patient and Addiction Specialist daily interaction.
- Integrates 12 step principles into programmatic counseling, and Case Management while maintaining a professional treatment focus.
- Assists in maintaining collaboration communication with referral sources, and managed care entities.
- Participates in unit performance assessment and improvement activities.
- Reassess client to determine progress toward treatment goals.
- Solidifies transfer/discharge plans and arrangement for clients which take into consideration the needs of all clients.
- Maintain collaborating communication with referral sources and agencies to assure optimum service to patients and the community.
- Completes 20 CEU’S per year (licensure requirement).
- Provides at least 20 percent case management services.
- Maintain a minimum of the company’s productivity requirements (100%) at all times.
- Other responsibilities and tasks as may be assigned or may arise.
Administrative
- Performs all pertinent treatment documentation in the patient record/ chart in prescribed time limits (24 hours).
- Provide administrative documentation in a timely manner.
- Follow-up with clients (or refer to Engagement Specialist for follow-up) regarding missed or cancelled appointments via phone calls and/or letters.
- Writes reports as required on progress of clients.
General
- Conduct all professional activity in an ethical manner.
- Conform to all applicable policies and procedures.
- In conjunction with other staff assist in identifying and eliminating any situation which jeopardizes the health and/or safety of clients, staff, or others who come in contact with community mental health center operations.
- Utilize literature, workshops and other training aids to continually improve clinical and administrative skills.
- In conjunction with supervisor, complete and implement Individual Professional Development Plan.
- Assist in assuring Edgewater Systems compliance with all related licensing and accreditation requirements.
Minimum Qualifications
- Education – Minimum of a Bachelors degree in a behavioral health field.
- Licensure/Certifications - Licensure preferred.

- Certification as a chemical dependency counselor
- Licensed addiction counselor (LAC)
- Licensed Clinical Addiction Counselor (LCAC)

- Experience – Minimum of 5 years experience preferred.
- Other - Possess a valid driver’s license.

- Furnish own inspected, automobile as needed for Center work and transportation of clients.
- A good driving record.
- Appropriate automobile insurance and valid license tags.
