Engage in comprehensive assessment of the referred child's ecology to determine family and system-identified strengths, needs, resources, stressors, and sources of support, to be used in care planning. Flexible hybrid work model, which includes meeting clients in a community- or home-based setting and choice to work from home or dedicated office space.
Monday through Friday position with flexible hours based on clients' needs.
Duties and responsibilities
- Conduct comprehensive assessments for OhioRISE care coordination program. Conduct mental health assessments for the Intensive Home-Based Treatment (IHBT) program. Administer certain standardized measures, appropriate to employee's licensure and certifications.
- Work cooperatively with the Intake Manager to schedule and process all referrals to Connections for the OhioRISE care coordination and IHBT programs.
- Availability for emergency or short-notice response to requests for assessment.
- On-call crisis support for Connections' clients assigned to Intake Specialist.
- Face-to-face crisis support and intervention with kids and families while in the community.
- Obtain Child and Adolescent Needs and Strengths (CANS) certification for OhioRISE care coordination and IHBT eligibility. Certification needs to be renewed annually. Must obtain within first 30 days of employment.
- Participate (and pass requirements where applicable) in all required trainings, including but not limited to; First Aid, CPR, Blood Borne Pathogens, and Standards of Practice.
- Participate in administrative and clinical supervision as requested by supervisor, staff development activities, team meetings, and staff meetings as scheduled or assigned.
- Ability to work respectfully with a culturally diverse population of clients and multidisciplinary teams.
- Complete assigned duties competently and within expected time frames.
- Utilize safe work practices for self and others.
- Demonstrate word processing skills and program navigation skills.
- As a representative of the agency and profession, present self with decorum and in dress that is appropriate to the day's activities.
- Align practice and work with the agency mission, vision, guiding principles, the trauma-informed Sanctuary Model®, the Neurosequential Model and the Re-ED philosophy.
- Adhere to PEP and ESC policies and procedures, as applicable.
