Salary for this position: $24.00-28.00/Hour
Essential Functions:
- Coordinate patient-centered, cost-effective care for OBAT patients.
- Hire, train, and supervise nonclinical OBAT staff.
- Conduct patient screenings, schedule appointments, and provide social support navigation, including benefits applications and referrals.
- Offer nonclinical administrative support to the OBAT team.
- Collaborate with providers and the healthcare team to manage patient registries, appointments, and outreach.
- Complete required paperwork for OBAT patients.
- Facilitate new patient orientation within a month of intake.
- Build relationships with community organizations to enhance service coordination and referrals.
- Support OBAT team with grant tracking, reporting, and external programming.
- Assist in updating OBAT policies and procedures.
- Perform additional duties as assigned, including attending meetings and trainings.
- Education in Public Health, Social Work or other related health and human services.
- Experience providing direct social services or similar experience.
