We are seeking an experienced and compassionate Patient Care Manager to join our team at Egan-Ochsner Home Health Northshore. In this role, you will oversee clinical services and provide nursing care to patients in their homes as prescribed by physicians and in compliance with applicable laws, regulations, and agency policies.
Key Responsibilities:
- Clinical Services Coordination: Provides clinical services within the scope of practice, as defined by state laws governing nursing practice, in accordance with the plan of care, and in coordination with other healthcare team members.
- Referral Management: Receives referrals, ensures clinician assignments, evaluates patient orders, and plots start of care visits.
- Benefits Determination: Coordinates determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
- Needs Assessment: Ensures patient needs are continually assessed and care rendered is individualized to patient needs, appropriate, and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
