Responsibilities
- Clinical Services: Provides clinical services within the scope of practice, as defined by state laws governing nursing practice, in accordance with the plan of care and in coordination with other healthcare team members.
- Referral and Assignment: Receives referrals, ensures appropriate clinician assignments, evaluates patient orders, and plots start-of-care visits.
- Benefits Coordination: Coordinates determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
- Patient Assessment: Ensures patient needs are continually assessed and care rendered is individualized to patient needs, appropriate, and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
Requirements
- RN Licensure: Current RN licensure in the state of practice.
- CPR Certification: Current CPR certification required.
- Driver's License: Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- Experience: LA: At a minimum, one year of clinical experience. RN licensure must have no restrictions.
