EHM Senior Solutions is seeking a compassionate and organized Healthcare Coordinator to support and oversee the care team at Brecon Village, including our licensed AFC Memory Support Center. Key Responsibilities
- Team Leadership: Provide daily oversight of care staff to ensure exceptional quality of care for residents.
- Staffing and Scheduling: Ensure sufficient staffing levels, support scheduler, and participate in the on-call rotation.
- Resident Advocacy: Address resident and family concerns with empathy and professionalism.
- Team Support: Liaise/Collaborate with multidisciplinary healthcare team to support resident care
- Care Team Collaboration: Facilitate daily huddles with the care team to ensure seamless coordination and communication.
- Care Analysis: Conduct resident assessments/evaluations and participate in care conferences.
- Training and Development: Assist in creating and delivering orientation and training programs to support care staff.
- Quality Initiatives: Contribute to quality improvement projects and maintain compliance with regulatory standards.
- Care Planning: Collaborate with the care team to create, update, and monitor individualized care plans for residents.
- Required:
- Active LPN license in the State of Michigan.
- Experience in assisted living or memory care settings.
- Strong understanding of Alzheimer’s, dementia, and progressive memory care needs.
- Preferred:
- Strong leadership, organizational, and communication skills.
