Onsite Enrollment Coordinator

Job Title: Patient Enrollment Coordinator Contract Duration: 3 Months (Potential for Permanent Placement) Commencement: Immediate Location: Andover, MA Qualifications: Minimum of 1 year experience in a healthcare environment in an administrative or patient-facing capacity. Demonstrated proficiency in face-to-face customer service, adept at addressing inquiries from patients and their caregivers. Excellent verbal and written communication skills, capable of effectively engaging with patients, caregivers, and medical professionals. Proactive and outgoing demeanor, motivated by achievement. Familiarity with iCare or similar care management platforms is advantageous. Responsibilities: Facilitate enrollment of eligible Medicare patients into CCM or RPM programs. Coordinate with on-site medical personnel to optimize patient engagement and implement enrollment strategies. Prioritize CCM and RPM patient enrollment, including providing education, demonstrating devices, addressing patient inquiries, and securing patient consent. Record patient consents and all relevant interactions in the care management system (iCare), ensuring accuracy and completeness. Perform additional assigned tasks as required.
