EIHAB HUMAN SERVICES, New Jersey Inc. - Medical Coordinator
The Medical Coordinator (MC) conducts medical appointments to ensure that the individual receives all the treatments as ordered by medical professionals. The Medical Coordinator will monitor health and orders by following up on the medical recommendations, treatments, referrals, indications, or protocols that doctors prescribe. In addition, the Medical coordinator will monitor medical records for follow-up all the medical appointments for each individual, check and count every week the medications and place orders in case of need, check medications administration records daily to ensure all medication has been administered and documented according to policy. The medical coordinator serves as a substitute management role when management is unavailable.
- Provide staff (Direct Care) coverage in the event of shortage.
- Meet with the manager weekly and as needed, schedule all client medical, dental, psychiatric, and all other clinical appointments, as needed.
- Ensure all recommended follow-ups occur, as noted on clinical summaries and nursing notes
- Ensure that all prescribed client medication stock is maintained, and stored properly
- Ensure that all First Aid, and personal hygiene supplies are available at all times and their inventory maintained.
- Must accompany clients on medical appointments in the community.
- At all times on duty must ensure that the clients are appropriately dressed and groomed, before leaving the residence.
- Must ensure that all client medications are administered as prescribed, and Medication Administration Records are accurately documented.
- Must ensure that bagged lunches and snacks brought into the community are appropriate to the client menus.
- Must ensure all menus are developed according to each clients’ prescribed diet
- Must ensure all adaptive equipment, medications, diets, and special needs have up to date prescriptions.
- Must ensure all medical calendars are up to date with the proper appointments
- Must ensure clinical recommendations by medical professionals are properly documented on the appropriate physician forms and communicated to other personnel via the critical log.
- Comply with all other duties which may be assigned by the management or administration.
High school diploma or equivalent is required, plus two years related work or life experience is required.
Job Type: Full-time
Pay: $40,705.60 per year
