Overview
We are seeking a dedicated and knowledgeable Health Coach to join our team. The ideal candidate will have a strong background in health coaching, patient assessment, and care plan development. This role is essential in guiding individuals towards achieving their health and wellness goals through personalized coaching and support. The Health Coach will work closely with patients, particularly in long-term care and acute care settings, to promote healthy lifestyle choices and improve overall well-being.
Responsibilities
- Develop individualized care plans tailored to each patient's health goals and needs.
- Conduct comprehensive patient assessments to identify health risks and areas for improvement.
- Provide ongoing health coaching sessions to support patients in achieving weight loss and other health objectives.
- Collaborate with the dietary department to ensure nutritional needs are met, including guidance on tube feeding when necessary.
- Educate patients on physiology knowledge relevant to their health conditions and lifestyle changes.
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
- Maintain accurate records of patient interactions, progress, and any changes in care plans.
- Stay informed about the latest trends in health coaching, nutrition, and wellness strategies.
Qualifications
- Proven experience in health coaching or a related field with a focus on patient assessment and care plan development.
- Strong understanding of long-term care practices and acute care environments.
- Knowledge of dietary systems and the ability to collaborate effectively with dietary departments.
- Familiarity with weight loss strategies and techniques for promoting healthy lifestyle changes.
- Excellent communication skills, both verbal and written, with the ability to motivate and inspire patients.
- Ability to work independently as well as part of a multidisciplinary team.
- A background in physiology or related sciences is preferred but not required.
