Role and Responsibilities
Case Coordination & Support
- Conduct thorough assessments to determine resident needs and eligibility for services.
- Develop and implement personalized support plans and programs based on assessments.
- Maintain communication with all external and internal members of the resident’s team (e.g., guardians, external case managers).
- Develop and maintain regularized cadence of scheduled Team Meetings – internal/external.
- Facilitate appointments for external medical needs, including PM&R Appointments and establishing residents with new providers – as needed.
- Maintain accurate and up-to-date records of resident interactions and services provided.
- Serve as the primary point of contact for residents, providing regular updates and addressing any concerns.
- Advocate on behalf of residents to ensure they receive the necessary support and resources.
- Ensure all activities comply with company policies and relevant regulations.
- Make weekly contact with residents to ensure that habilitation needs are being addressed.
- Resident coordination and money management.
- Manage the daily deployment of staff on all shifts to ensure coverage and maintain staffing ratios.
- Ensure coordination, staffing, and transportation for off-campus medical appointments.
- Develop and support engagement and leisure outings.
- Maintain AFC book and other Resident information on the unit, aligning with corporate process and identified best practice.
- Complete Key Performance Indicators (KPI) for residential staff as set by Residential Director
- Ensure that Daily Progress Notes (DPN) are completed daily and turned in weekly
- Maintain billing utilization requirement as set by leadership.
- Ensure that all documentation is completed within 24 hours of delivery service.
- Ensuring that all insurance requirements regarding authorization and payment of services are completed timely.
- Authorization and Residential script management
- Ensure that Medicaid requirements are managed and maintained in a manner that ensures compliance and continuity of care.
- Bachelor’s degree (or higher) in Social Work, Psychology, Human Services or related field.
- Skilled in the provision of evidence-based treatments.
- Minimum of 2 years of experience in a similar role, preferably in case management or social services.
- Commitment to professional boundaries.
- Proficient in Microsoft Office Suite – Word, Outlook, Excel, PowerPoint
- Advanced skill set as it relates to time management, meeting deadlines, and communication.
