Job Summary
We are seeking a dedicated and compassionate Case Manager to join our healthcare team. Duties

- Collaborate with multidisciplinary teams in hospital settings, including Level II Trauma Centers and pediatrics departments.
- Provide support for patients receiving hospice care and home health services.
- Utilize medical terminology effectively to communicate with healthcare professionals.
- Offer mental health counseling as needed, ensuring a holistic approach to patient care.
- Respond to emergencies and coordinate emergency medicine interventions when necessary.
- Maintain accurate documentation of patient interactions, care plans, and progress notes.
Experience
- Previous experience in case management or a related field is preferred.
- Familiarity with care plans, data collection techniques, and medical terminology is essential.
- Experience working in hospital environments or with hospice care services is a plus.
- Background in emergency medicine or pediatrics will be beneficial for this role.
- Strong interpersonal skills with the ability to provide mental health counseling as needed.
