Job Summary
We are seeking a dedicated and compassionate Wound Care Nurse to join our healthcare team. This role involves assessing, planning, implementing, and evaluating care for patients with complex wounds in various settings, including assisted living facilities. The Wound Care Nurse will work closely with patients, families, and interdisciplinary teams to ensure optimal healing outcomes.
Responsibilities
- Conduct thorough assessments of patients’ wounds and overall health status.
- Develop individualized care plans based on assessment findings and patient needs.
- Perform wound care treatments using aseptic techniques to prevent infection.
- Administer medications as prescribed and monitor patients for adverse reactions.
- Execute IV insertion and phlebotomy procedures when necessary.
- Collaborate with healthcare professionals in discharge planning to ensure continuity of care.
- Provide education to patients and families regarding wound care management and preventive measures.
- Document all patient interactions, treatments, and progress accurately in medical records.
- Support patients with disabilities and provide appropriate care for those with dementia.
Experience
- Current nursing license with experience in wound care preferred.
- Background in assisted living or long-term care settings is advantageous.
- Familiarity with internal medicine practices related to wound healing.
- Proven ability to work effectively with individuals with disabilities.
- Experience in discharge planning processes is a plus.
- Strong communication skills and the ability to work collaboratively within a team environment.
