Job Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. Responsibilities
- Deliver direct patient care under the supervision of a registered nurse or physician.

- Provide support in triage situations, ensuring patients receive timely care.
- Assist in health coaching and education for patients and their families regarding treatment plans and wellness strategies.
- Collaborate with multidisciplinary teams to develop and implement individualized care plans for patients.
- Maintain accurate patient records and documentation in compliance with healthcare regulations.
- Support pediatric patients and those with developmental disabilities through specialized care approaches.
- Ensure a safe and clean environment for patients by adhering to infection control protocols.
Experience
Candidates should possess the following skills and experience:
- Previous experience in medical office settings or home care is preferred.
- Familiarity with hospital medicine practices is advantageous.
- Proven ability to provide compassionate patient care across diverse populations, including pediatrics and individuals with developmental disabilities.
- Experience in primary care settings is a plus.
