Job Summary

Duties
- Conduct thorough assessments of patients' health and medical histories.
- Provide education and support to patients regarding reproductive health, prenatal care, and family planning.
- Assist in the management of patient care plans, including case management and discharge planning.
- Document medical information accurately and maintain up-to-date medical records using Cerner or similar electronic health record systems.
- Collaborate with physicians and other healthcare professionals to ensure comprehensive patient care.
- Implement utilization management strategies to optimize patient outcomes.
- Offer health coaching to empower patients in making informed decisions about their health.
- Stay current with developments in women's health, including physiology knowledge and coding for medical documentation.
Requirements
- Registered Nurse (RN) license with experience in obstetrics and gynecology preferred.
- Strong knowledge of case management principles and practices.
- Familiarity with primary care protocols and senior care considerations is a plus.
- Proficient in medical documentation practices, including coding standards.
- Excellent interpersonal skills with the ability to communicate effectively with patients and their families.
