Overview

Duties

- Collaborate with healthcare professionals to develop and implement individualized care plans for patients with complex medical needs.
- Perform various medical procedures such as wound care, IV therapy, and catheterization while adhering to aseptic techniques.
- Educate patients and their families about health conditions, treatment options, and post-discharge care instructions.
- Maintain accurate and timely documentation of patient assessments, interventions, and outcomes using eClinicalWorks or similar electronic health record systems.
- Participate in quality improvement initiatives and adhere to infection control protocols to ensure a safe environment for patients.
Skills

- Experience in assisted living environments is advantageous.
