Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. In this pivotal role, you will be responsible for coordinating patient care, ensuring optimal health outcomes, and facilitating communication among patients, families, and healthcare providers. Responsibilities
- Conduct comprehensive assessments of patients’ medical histories and current health status.
- Develop individualized care plans based on assessment findings and patient goals.
- Coordinate care across multiple disciplines and settings, including hospital stays and outpatient services.
- Monitor patient progress and adjust care plans as necessary to meet changing needs.
- Educate patients and their families about disease management, treatment options, and available resources.
- Ensure compliance with HIPAA regulations and maintain confidentiality of patient information.
- Collaborate with healthcare teams to facilitate effective communication regarding patient care.
- Advocate for patients’ needs within the healthcare system to ensure they receive appropriate services.
Experience
- Current Registered Nurse (RN) license in good standing.
- Experience in utilization management, infectious disease care, behavioral health, medical-surgical nursing, or emergency room settings is highly desirable.
- Knowledge of geriatrics and experience working in a Level II Trauma Center is a plus.
- Strong understanding of physiology and the ability to apply this knowledge in clinical settings.
- Proven ability to work collaboratively within a multidisciplinary team environment.
- Excellent communication skills, both verbal and written, with a focus on patient education and advocacy.

Pay: $65.00 - $73.00 per hour
