We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. Responsibilities
- Assess patients’ health status and develop individualized care plans based on their medical needs.
- Coordinate and manage patient care across multiple disciplines, ensuring seamless transitions between different levels of care.
- Collaborate with healthcare providers, including physicians, social workers, and therapists, to facilitate comprehensive patient management.
- Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes.
- Provide education and support to patients and their families regarding treatment options, resources, and follow-up care.
- Document all patient interactions and maintain accurate records in compliance with healthcare regulations.
- Advocate for patients’ needs within the healthcare system to ensure they receive appropriate services.
- Participate in quality improvement initiatives to enhance patient care delivery.
Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Experience in Medical-surgical nursing, Behavioral health, or Geriatrics is preferred.
- Previous experience in Case Management or similar roles is highly desirable.
- Strong clinical background in settings such as OR, Hospital, PICU, or Inpatient care.
- Excellent interpersonal skills with the ability to communicate effectively with patients, families, and healthcare professionals.
- Ability to work independently as well as collaboratively within a multidisciplinary team.
- Strong organizational skills with attention to detail in managing multiple cases simultaneously.
- Proficient in electronic medical record (EMR) systems and documentation practices.

Pay: $65.00 - $72.00 per hour
