Job Summary
We are seeking a compassionate and dedicated Registered Nurse - Home Health to provide high-quality care to patients in their homes. The ideal candidate will possess a strong clinical background and the ability to work independently while delivering personalized nursing care. Duties
- Conduct comprehensive assessments of patients' health status and develop individualized care plans.
- Administer medications and treatments as prescribed, ensuring adherence to aseptic techniques.
- Monitor patient progress and make necessary adjustments to care plans based on clinical findings.
- Provide health education and coaching to patients and their families, fostering a supportive environment for recovery.
- Collaborate with multidisciplinary teams, including case management, to coordinate patient care effectively.
- Document all patient interactions, assessments, and interventions accurately in electronic health records (EHR) systems.
- Assist with specialized procedures when necessary, including neonatal ventilator management and care for pediatric patients in PICU settings.
- Participate in quality improvement initiatives to enhance patient outcomes and service delivery.
Skills
- Proficiency in case management practices and MDS documentation.
- Experience in emergency room (ER) settings or critical care (CT) is highly desirable.
- Strong understanding of health coaching principles to support patient education.
- Familiarity with clinic operations and home health care protocols.
- Excellent communication skills, both verbal and written, for effective patient interaction.
- Ability to work independently while managing time effectively in a home health environment.
- Knowledge of infection control practices and aseptic techniques.
