What are the responsibilities and job description for the Community Health Outreach Specialist position at El Concilio California?
THE POSITION: Under the Supervision of the Outreach Supervisor, the Community Health Outreach Specialist will be responsible for promoting and bringing awareness to various programs, initiatives, and services available through El Concilio California. This position will conduct outreach and serve as trusted community member who serves as a link between health and social services.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Adhere to all El Concilio California policies and procedures.
Network, plan, and establish opportunities to promote El Concilio Californias programs and initiatives throughout San Joaquin/Stanislaus County.
Build and maintain relationships with community stakeholders, including resource centers, medical facilities, and advocacy groups to bring additional resources to our members' attention.
Maintain accurate documentation of program updates using multiple digital database.
Target underrepresented communities and conduct outreach by attending local community events and door-to-door canvassing.
Conducts regular friendly outreach calls to community members to track continuity of care and anticipate concerns or questions.
Aim for the goal of 500 enrollments through Sonja Cares.
Aim for the goal of 50,000 successful contacts with community members to educate and assist with MediCal enrollment.
Aim for the goal of 600 individuals to be informed regarding COVID-19/Flu Vaccines updates.
Advocate for patients to access necessary services, such as housing, food assistance, transportation, and mental health resources.
Assist patients in navigating the healthcare system, including follow-up care, specialty appointments, and referrals.
Become familiar with multiple public health websites and database to complete data input.
Maintain a calendar of all community events planned throughout San Joaquin/Stanislaus county.
Develop and distribute informational learning materials to community members.
Other duties as assigned.

18 years of age or valid work permit
Valid Californias Drivers License is required

EDUCATION/EXPERIENCE: A Bachelors Degree in Public Health or related field and 1-3 years of experience conducting outreach is highly preferred; an Associates Degree in Public Health or related field and 2-4 years of experience conducting outreach is highly preferred; or A high school diploma/GED Equivalency and 3-5 years of experience
LANGUAGE SKILLS: Must be able to communicate clearly; read and write in English and Spanish language is required.

Successful completion of Californias Sexual Harassment Prevention Training
Successful completion of HIPAA Training
Successful completion of Workplace Violence Prevention Training

WORK ENVIRONMENT: The noise level in the work environment is moderate to high. Primary work environment is an office cubicle space and public settings. The employee is regularly required to work indoors within an office setting and outdoors in moderate weather.
