Care Team RN

El Dorado Community Health Centers, a FQHC (Federally Qualified Health Center) offering Primary Care Services with locations in Placerville and Cameron Park, is seeking a full-time Care Team RN. As a non-profit community health center EDCHC provides Medical, Dental, Optical, Pharmacy, Behavioral Health and SUD Treatment Services to underserved populations.

We are looking for a qualified Registered Nurse to join our nursing team. The Ideal candidate will have direct patient-care experience in a community health or primary care setting. RN candidate should have a desire to work with underserved communities and a passion for health education and primary care.

This position offers the ability to see patients both independently and as a vital team member in an integrated primary care model. Nurses in our clinics work to the top of their licensure to provide patient-centered care and gain experience in case management, health coaching, triage, and furthermore are able to improve patient care through participation in and development of quality improvement initiatives.

Experience:

• Associate or BS Nursing degree.
• Active California RN License in good standing.
• Current AHA CPR Certification.
• 2 years of experience as an RN in any setting.

Education and Licensing Requirements:

• Current, valid, unsuspended State of California license as a registered nurse
• Current, valid, unsuspended State of California Driver’s License
• Current CPR certification.

OTHER

• Experience in a community clinic or primary care setting preferred.
• Familiarity or proficiency in using eCW Electronic Health Records desired.
• Experience with nursing care coordination/case management.
• Bilingual in Spanish highly desired.
• Excellent verbal and written skills.
• Strong enthusiasm and willingness to pitch in whenever needed.
• Ability to work independently, with written protocols.
• Ability to be sensitive to ethnic and cultural differences and their relevance to health care.
