Responsibilities: The Pre-Exposure Prophylaxis (PrEP) Navigator provides intensive care coordination, support and services to HIV negative individuals who require assistance in accessing and remaining in PrEP care. This individual will coordinate with the Prevention team to initiate access to PrEP care, alongside working with the Medical and Pharmacy teams to provide assistance to new and existing PrEP patients.
This individual will implement a client-centered approach with individuals throughout the El Proyecto Del Barrio Organization. This position will assist PrEP clients with services such as, but not limited to, health insurance enrollment, PrEP provider identification, appointment scheduling, follow-ups, and Health Preventative Education.
- Provide support to HIV negative individuals who are interested in starting PrEP and those already utilizing PrEP care.
- Complete screenings to identify readiness for PrEP and other barriers to care.
- Work with client to identify position solutions and Health Education.
- Assess benefits and insurance, and if needed, provide assistance to access payor programs.
- Provide PrEP disease prevention and PrEP adherence education and counseling.
- Design individualized program reports, narrative activity descriptions, reviews and other written records for evaluating and analyzing program activities.
- Make referrals for external services needed to support PrEP care and follow-up to ensure service delivery and adherence.
- Maintain appropriate patient files and timely documentation of services.
- Serve as a fully functioning member of the Prevention team, able to participate in daily activities and functions to maintain overall EPDB operations.
- Provide quarterly reports to management and funding sources when needed.
- Establish working relationships with appropriate community resources in the area, including those providing mental health and social services to the target population.
- Perform other duties assigned by the Clinic Manager.
Qualifications
Requirements:
Demonstrate ability to work independently and in a team setting. He/she should feel comfortable with public speaking and have the ability to provide services with cultural competence. Ability to drive to off-site locations and work occasional evenings and weekends. Community-based work required. Must submit to and pass a criminal background check clearance, drug test, and fitness for duty as pre-condition for employment. Must be able to provide proof of vaccinations for COVID-19 with booster, MMR, TDAP, and Hepatitis B. Must provide a recent TB test. BLS/CPR certification required.
Education:
Bachelors Degree in social work, counseling, nursing, health education or related field. Two (2) years of experience working in HIV prevention or care, medical care systems, diverse populations, and/or underserved communities, preferred.
Salary : $23 - $36
