The primary goal of the El Rio Health
Dental Health Home Coordinator is to support El Rio’s Mission of providing comprehensive, quality health care that is affordable and accessible to all who may have healthcare needs, by successfully performing the primary essential functions. \nThe following functions and responsibilities will contribute to El Rio Health to achieve its KEY RESULTS : \n \n\nWorld Class Experience for staff and patients\nHealthier patients and employees\nPositive Financial Result\n\n \nEssential Job Functions : \n\nServes as the primary point of contact between patient, medical and dental treatment teams, and internal / external stakeholders to ensure continuity of care.\nSupports patient in navigating initial health center dental appointment to ensure positive patient experience.\nFacilitates / assists patients in the new patient intake process.\nMaintains knowledge of the link between oral health and common chronic health conditions, behavioral health disorders and management techniques to enhance patient “self-care.”\nProvides ongoing direct support services to patients with goal of improving and / or maintaining whole health.\nServes as advocate for patients to ensure their oral health needs are met and to role model self-management skills.\nMaintains regular contact with assigned dental patients to monitor progress, identify any service needs and communicate concerns to medical and dental treatment team.\n\n \n\nHelps patient with financial responsibility review of oral health treatment plan and presents oral health treatment plan to the patient.\nFacilitates treatment plan presentation to patient.\nEducates patient on treatment options.\nReviews financial responsibility with the patient.\nFacilitates referrals, assists in scheduling appointments, assesses eligibility for / secure enrollment in appropriate health insurance coverage for patient and discuss financial arrangements with patient.\nSupports and navigates patients in securing and enrolling for specialty dental services as needed.\nPartners with assigned medical and dental provider(s) and treatment team to coordinate care and follow up on any urgent needs of patients.\nAssists patients with enrollment into and usage of the patient portal.\nFacilitates communication and coordination with specialty dental services and ensures appropriate planning on behalf of patient.\nMonitors and evaluates data related to gaps in dental care of assigned patient panel; and utilizes analytical software to manage population health data of assigned patient panel.\nMaintains a clean, safe, and hygienic work environment in compliance with all Policies and Procedures including but not limited to work areas, workstations, examination rooms, hand washing, infection prevention and control etc. for this position.\nDemonstrates an understanding of and proficiency with the application of all compliance and reporting requirements respective to Joint Commission Certification (JCC) standards.\n\n \nMinimum Education and Experience : \n\nAssociate of Arts degree in Health Care, Human Services, or related field from an accredited college; Three (3) years’ experience with direct patient contact either in a dental clinic or health care setting \nThree (3) years’ experience working with Electronic Health Records (EHR).\n\n \nIf applicable, equivalent combination of education and experience may be considered, and must be directly related to the functions and responsibilities of the job.\n \nRequired Licenses, Certifications, and Registrations : \n\nCertificate as a Dental Assistant or a Medical Assistant from an accredited program.\n\n\nCurrent certification in Basic Life Support (BLS) for health care providers. \nLevel I fingerprint clearance card : current valid and in good standing or have applied for it within seven working days after beginning employment.\nEmployees in this position are required to have reliable transportation that can meet any operational, reassignments of the organization during the workday. If an employee is driving during work hours, the employee is required to possess a valid driver’s license and must be in compliance with Arizona vehicle insurance requirements.\n\nPreferred Education, Experience, Skills, Abilities : \n\nCertified Application Counselor (CAC) Program. \nBachelor’s Degree in Health Care, Human Services, or in a related field from an accredited college.\nFive (5) years of experience working with Electronic Health Records (EHR).\nFive (5) years of experience working with direct patient contact either in a dental clinic or health care setting.\n\nBilingual (English / Spanish) with the ability to speak, read and write in both languages.\nEl Rio Health is a diverse and inclusive organization.
