SUMMARY: A Registered Nurse is a member of the healthcare team who has successfully completed a State Board of Nursing approved education program and successfully obtained a license to practice professional nursing in the Commonwealth of Pennsylvania. The Registered Nurse practices professional nursing by providing nursing diagnosis and nursing treatment of human responses to actual or potential health problems through such services as case findings, health teaching, health counseling, provision of care supportive to or restorative of life and well-being, and executing medical regimens as prescribed by a licensed physician or dentist.
The Registered Nurse is responsible for supervising and directing the work efforts of licensed and unlicensed personnel on an assigned unit, the delivery of quality nursing care and treatments, the maintenance of medical records and the coordination of individualized services to residents. The Registered Nurse also provides direct nursing care to residents as authorized by their license status.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties maybe assigned.
- Supervises the delivery of nursing care given to residents by licensed and unlicensed nursing staff.
- Functions as a role model for the nursing staff on the unit.
- Provides staff education as needed to ensure quality nursing care.
- Prepares and coordinates staff assignments.
- Reviews resident care needs with assigned staff.
- Provides counseling to staff as needed.
- Delivers care to residents by:
- Collecting complete and ongoing data to determine nursing care needs.
- Analyzing health status of individuals and families and comparing the data with norms when possible in determining nursing care needs.
- Identifying goals and plans for nursing care.
- Carrying out nursing care actions which promote, maintain, and restore the well-being of individuals.
- Involving individuals and families in their health promotion, maintenance, and restoration.
- Evaluating the effectiveness of the quality of nursing care provided.
- Administering and evaluating the effect of medications.
- Administering and evaluating the effect of treatments.
- Delivering direct resident care following the Residents Individualized Care Plan. This includes, but is not limited to: assisting with meals, bathing, personal hygiene, oral hygiene, incontinence care, toileting, positioning, transferring, splint application, etc.
- Assessing residents as indicated by their status in accordance with facility procedures and standards of nursing practice.
- Documenting care delivered and assessments according to facility procedures.
- Making rounds on a regular basis to observe and monitor care being rendered.
- Assisting with physician rounds as needed.
- Ordering supplies, medications, and equipment as per facility policies and procedures.
- Communicates with the Registered Nurse in charge, the residents health care team members, and the Residents Representative to seek guidance when:
- The residents care exceeds the Registered Nurse scope of practice.
- The residents care needs surpass the RNs knowledge, skill, or ability.
- There is a significant change in condition (i.e. improves or deteriorates).
- The resident is not responding to therapy.
- The resident becomes unstable or needs immediate assistance.
- Accepts written and verbal orders for medications and therapeutic treatment from an authorized practitioner in accordance with facility policy.
- Questions any order that is perceived as unsafe or contraindicated for the resident or which is not clear and shall raise the issue with the ordering practitioner or the professional nurse in charge if the ordering practitioner is not available.
- Transcribes orders to the residents medical record per facility policy.
- Provides venipuncture: intravenous fluids when:
- The procedure has been ordered in writing for the resident by an authorized licensed practitioner.
- The Registered Nurse has had instruction and supervised practiced in administering parenteral fluids or medications into the vein.
- The intravenous fluid or medication to be administered is the fluid or medication specified in the written order.
- An accurate record is made concerning the following:
- The time of the injection
- The medication or fluid injected
- The amount of medication or fluid injected
- Reactions to the fluid
- Supervises Licensed Practical Nurses in the provision of venipuncture/intravenous fluid administration.
- Seeks instruction and supervision when implementing new or unfamiliar nursing practices or procedures.
- Assists in orientation of new staff.
- Attends meetings as assigned.
- Participate in Quality Assurance/Performance Improvement activities as assigned.
- Adhere to facility policies & procedures for nursing practice.
- Understand and adhere to 49 Pa. Code 21.18 Standards of nursing conduct.
- Understand and comply with 49 Pa. Code 21.11 - 21.16 Responsibilities of the Registered Nurse.
- Understand and comply with State and Federal regulations for care of residents in nursing facilities.

SUPERVISORY RESPONSIBILITIES:
Directly supervises licensed and unlicensed nursing staff on his/her assigned nursing unit. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

EDUCATION and/or EXPERIENCE:
Registered Nurse
Must maintain current Basic Cardiopulmonary Life Support (BCLS)
Must have completed a Commonwealth of Pennsylvania State Board of Nursing approved IV therapy program or have documentation of completion of supervised venipuncture and intravenous fluid administration.
LANGUAGE SKILLS:
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence such documentation in medical records. Ability to speak effectively before groups of residents and families or employees of the organization.
MATHEMATICAL SKILLS:
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
REASONING ABILITY:
Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
CERTIFICATES, LICENSES, REGISTRATIONS:
Must possess a valid Pennsylvania nursing license.
OTHER SKILLS and ABILITIES:
Strong organizational and interpersonal skills.
Conforms to established work hours and maintains an acceptable work attendance record.
