What You’ll Do:
As an ADvantage Case Manager, you will:
- Conduct initial assessments to identify members' needs.
- Develop and implement personalized care plans.
- Provide monthly monitoring
- Coordinate services to maintain members' safety and independence.
- Participate in in-service programs and ADvantage training.
Qualifications:
To excel in this role, you’ll need:
- A bachelor’s degree and at least one year of paid professional experience with aging or disabled populations.
- A valid driver’s license, reliable transportation, and proof of insurance.
- Willingness to travel (mileage will be reimbursed)
- Experience with the ADvantage Program (preferred).

Pay: $18.00 - $22.00 per hour
Expected hours: 15 – 40 per week
