Job Description:
As a Restorative Nurse, you will play a pivotal role in promoting wellness and independence among our residents. You will collaborate closely with our interdisciplinary team to develop and implement individualized care plans tailored to each resident's needs. Your responsibilities will include:
- Conducting comprehensive assessments to identify residents' functional abilities and limitations.
- Designing and overseeing personalized restorative care programs aimed at maintaining or improving residents' physical and cognitive functioning.
- Providing hands-on training and support to nursing staff and caregivers in implementing restorative interventions.
- Monitoring residents' progress and adjusting care plans as needed to optimize outcomes.
- Collaborating with therapists, physicians, and other healthcare professionals to coordinate holistic care for residents.
- Documenting all interventions, progress, and outcomes accurately and timely.
- Valid nursing license (RN or LPN) in Illinois.
- Proven experience in geriatric nursing or long-term care settings is required.
- Strong understanding of restorative nursing principles and practices.
